• Sometimes things happen, such as insufficient sample volume or blood hemolysis. In these cases, we believe in fair cost-sharing for replacement kits: HealthieOne covers 50% and you cover 50%. The prices shown below reflect 50% of the actual cost.
• When issues are clearly caused by shipping carrier delays or damage during transit, HealthieOne takes full responsibility and covers the entire replacement kit cost.
• If you choose to have a professional phlebotomist collect your blood sample, we reduce the kit prices to offset the cost of phlebotomist services.

For samples not yet sent due to insufficient volume or missed same day shipping:

• Partial Replacement Kit for Self-Collection: $60
• Or Partial Replacement kit for Phlebotomy: Free kit but you pay phlebotomist directly. 

For samples that have been sent but are not usable:

• Full Replacement kit for Self-Collection: $100
• Or Full Replacement Kit for Phlebotomy: $60 + you pay phlebotomist directly. 

For return shipping delays:

• Full Replacement Kit for Self-Collection due to Delayed Delivery: Free kit.

• These independent phlebotomy providers across the country are available to collect samples for HealthieOne and send your samples to our lab per our protocols.

• You have the flexibility to choose whichever provider best meets your needs based on location, availability, and pricing.

• Please note that you will be responsible for paying the phlebotomist directly for their collection services at their standard rates and scheduling your appointment with them.
