For samples not yet sent due to insufficient volume or missed same day shipping:

- Or order a Partial Replacement kit for Phlebotomy: we send the kit at no cost and absorb the loss. You'd just pay the phlebotomist directly. Still 2 small vials, you choose any certified phlebotomist.
For samples that have been sent but are not usable:
- Both vials are not not usable: Order a Full Replacement kit for Self-Collection: $100
- One of the two vials is not not usable: Order a Partial Replacement Kit for Self-Collection: $60
- Or order a Full Replacement Kit for Phlebotomy: $60 + you pay phlebotomist directly. Still 2 small vials, you choose any certified phlebotomist.
When shipping delays make your samples non-viable:
- Carrier delays: Order a free Full Replacement Kit for Self-Collection due to Delayed Delivery.
- You missed drop-off time or drop-off at non-FedEx Office location: Order a Full Replacement kit for Self-Collection: $100
When a phlebotomy service mishandles your samples and makes them non-viable:
- The phlebotomy service cover the cost of Full Replacement Kit for Phlebotomy + redraw free of charge.
