Consent Agreement

Age Requirement
By proceeding with this service, you confirm that you are at least 18 years old. If you are under 18 years of age, you are not permitted to use this service.

1. Confidentiality and security of HealthieOne test results and consumer information:

I understand that I must be at least 18 years old to use this service. I understand that my medical records will only be released to my healthcare providers upon my written authorization, to the extent necessary for my care or reimbursement purposes as required by law. I understand that I have the right to withhold consent for sharing my information with my healthcare provider by selecting the "No" option in the provided questionnaires.

I recognize that, aside from the exceptions mentioned above, my HealthieOne test results and my information will not be sold to any third parties. Gigantest Laboratory, which creates, processes and analyzes my test, is a CLIA-certified and CAP-accredited facility that adheres to the highest standards of clinical testing.

I acknowledge that anonymized and de-identified data may be utilized to enhance a human health database, which will help Gigantest improve health testing and diagnostics while maintaining consumer confidentiality.

2. Agreement to pay for services:

I agree to pay in full for all services rendered if I decide to purchase HealthieOne tests. I acknowledge that payment for HealthieOne tests is due at the time of purchase.

I understand that it’s my responsibility to contact my healthcare providers and my insurance to determine whether my insurance covers the cost of HealthieOne tests and related services.

I understand that I am responsible for submitting the HealthieOne itemized receipt to my insurance company for potential reimbursement. Any insurance reimbursement will be paid directly to me. I acknowledge that regardless of insurance reimbursement status, I remain responsible for paying HealthieOne in full at the time of purchase.

I acknowledge that if I do not pay my bill, my account may be sent to a collection agency. In such cases, I agree to pay a reasonable attorney’s fee of twenty-five percent (25%) of the outstanding balance at the time of referral, along with any associated court costs and private process server fees.

3. Consent to be contacted by phone and email:

I authorize Gigantest, its affiliated entities, or their collection agencies to contact me at any telephone number or email address associated with my account regarding their services. Methods of contact may include prerecorded voice messages, automatic dialing devices, emails, and/or text messages.
