Explanation: 3-OH-octadecadienoylcarnitine accumulates during the breakdown of unsaturated fatty acids for energy. Elevated levels indicate problems with fatty acid metabolism including conditions like long-chain 3-hydroxyacyl-CoA dehydrogenase (LCHAD) deficiency. When these enzymes don't work properly, the body cannot efficiently convert fats into energy, causing hydroxylated fatty acid byproducts to build up in the blood. This affects energy production especially during fasting, illness, or periods of increased energy demand.
Test Purpose: Detect long-chain fatty acid oxidation disorders, particularly LCHAD deficiency.
